About The Position

The Case Manager will perform initial assessments, provide ongoing supportive services both in the field and in the office and maintain documentation requirements for justice involved clients with histories of mental illness, substance use and homelessness.

Requirements

• Ability to oversee a caseload of 15 clients
• Ability to provide a collaborative assessment of needs, create and implement service plan, monitor progress and evaluate services provided to the individual client
• Proficient in Microsoft Excel, Word, and Outlook with a working knowledge of PowerPoint
• Excellent written and verbal communication skills
• Excellent interpersonal skills with ability to interact culturally, linguistically, and diplomatically with diverse internal and external individuals
• Physical ability to travel throughout the 5 boroughs via public transportation in accordance with client location(s)
• Individuals must be able to climb several flights of stairs to perform home visits when needed

Nice To Haves

• Associate’s Degree in Human Services or/and five years of experience working with special needs population
• High School Diploma and eight years of experience working with special population
• Bachelor’s degree in human services is preferred with at least 2 years of experience with the target population
• Relevant personal experience is a plus
• Individuals who have successfully made the transition from incarceration and/or substance abuse into a stable, productive lifestyle in the community

Responsibilities

• Conduct intake needs assessments on clients eligible and accepted for housing services
• Create individualized service plans and Outcome Stars for each client
• Document all client services, including successful and unsuccessful outreach attempts
• Conduct crisis intervention with clients, as needed
• Assess client’s need for temporary rental subsidy and furniture/moving allowance
• Assist clients in obtaining all necessary documentation to facilitate benefits application process
• Assist clients with completion of and submitting client’s application to appropriate benefits provider agency (e.g., HRA, Social Security)
• Act as a liaison to representatives of benefits administrations
• Act as liaison in conjunction with property management, between client and landlord, to mediate any landlord-tenant issues
• Provide ongoing counseling, supportive services needed to achieve goals stated in client service plan and Outcome Star
• Travel across the 5 boroughs on mass transportation as assigned and as needed to provide services to assigned participants
• Perform all other duties as assigned
