About The Position

Northeast Family Services is hiring Case Managers throughout New Hampshire. The Case Manager reports to the Clinical Supervisor and works with the clinical team as a liaison among the treatment team, clients, families, and collateral service providers. The role supports children and families through in-home and community-based services in accordance with referrals from DCYF. The position requires flexible availability, including early evenings, because most client visits occur in the afternoon or early evening and schedules depend on client needs. The Case Manager participates in an on-call rotation to provide phone support to families as utilized, transports clients, and documents clinical contacts and other required information through an Electronic Health System. The posting advertises a $500 stay-on bonus after six months and compensation of up to $50,000.

Requirements

Bachelor's degree with a major in social work, counseling, psychology, or a related human services field.
Valid driver's license and reliable, consistent access to a vehicle.
Authorization to work in the United States; the employer cannot sponsor or take over sponsorship of an employment visa at this time.

Flexible availability, including early evenings; schedule and hours are subject to client needs.

Nice To Haves

At least two years of experience working with children, youth, and families in a home or community setting.

Responsibilities

Maintain an assigned caseload and ensure proper implementation of in-home services in accordance with DCYF referrals.
Work collaboratively with DCYF on referral requests, treatment plans, treatment meetings, and regular service-specific documentation.
Develop and collaborate on treatment plans with measurable, attainable goals in partnership with families and DCYF.
Complete service-delivery documentation, including assessments and reports.
Report progress to the court system through in-person appearances and written reports as requested.
Provide parent education and supervised visitation to children, adults, and families in home and community-based settings.
Provide parent coaching and support, including effective parenting strategies and guidance on children's healthcare and educational needs, developmental milestones, meal preparation, and budgeting.
Enforce DCYF-identified supervised visitation expectations to ensure child safety.
Coordinate with DCYF, the New Hampshire court system, schools, behavioral health providers, and other treatment-team members as needed.
Assess client risk on an ongoing basis and consult with the Clinical Supervisor about safety concerns.
Transport clients, including children and adults, and install car seats and booster seats as required.
Participate in an on-call rotation and respond by phone to provide family support as utilized.
Document all clinical contacts and required documentation in the Electronic Health System.
Perform other duties as assigned.
