About The Position

The Salvation Army is seeking a Case Manager to counsel and assist homeless and underprivileged people and coordinate Emergency Solutions Grant Program outreach and services. This role involves working closely with various agencies to provide social services, manage client intake and assessments, determine eligibility for services, and input data into relevant systems. The Case Manager will also perform assessments, make referrals, conduct home visits, ensure housing habitability, assist with grant management, and participate in community outreach and disaster response efforts.

Requirements

  • Bachelor's Degree in Social Work or equivalent OR; Minimum two years of experience in social work or related field
  • Valid Guam driver's license
  • Prioritize multiple work tasks to meet tight deadlines
  • Travel off-island as necessary
  • Maintain confidentiality
  • Maintain regular and punctual work attendance

Responsibilities

  • Works closely with federal and local governmental agencies and non-profit and private charitable organizations to provide social services to the homeless, underprivileged, and those in crisis situations.
  • Schedules and coordinates Family Assistance clientele's intake, assessments, and monthly case management.
  • Determines eligibility of candidates for service with oversight and approval of the Corps Officers.
  • Input data gathered from client intake into the Homeless Management Information System (HMIS), WellSky, and other reports.
  • Perform assessments for the homeless, underprivileged persons, and families in crisis situations and assist them with connecting/referrals to agencies or other organizations accordingly.
  • Determines needs and provides services and referrals per directives and policies.
  • Makes home visits to ensure the well-being of clients.
  • Ensure units meet the habitability criteria before households sign a lease agreement.
  • Assists in the management of governmental grant requirements and expenditures of grant funds.
  • Participates in Christmas distribution, Point in Time count activities, and community outreaches.
  • Is a member of the Disaster Response Team and supports relief efforts in the aftermath of disasters.
  • Coordinates with grant funders and DHQ finance and social services.
  • Prepare check requests for clients' rental payments or deposits, monitor fund balances, and prepare financial reports and budgets.
  • Prepare reports to be shared at Advisory Board meetings and department meetings.
  • Assist the food pantry operations.
  • Perform other duties as assigned by the Corps Officers.
