About The Position

The successful candidate will be expected to serve as the primary contact for all Fiscal Intermediary related matters associated with their caseload. This role involves attending required internal and external job-related trainings, obtaining required documentation from internal/external customers, and informing the supervisor in a timely manner if requests for documentation are not responded to. The Case Manager will act as a liaison between the Participant/Designee/Circle of Support (COS) and internal SD departments to identify and resolve any issues with reimbursements and/or staffing associated with the Participant budget. A key responsibility is completing audits of their own caseload to ensure quality service delivery and compliance with all SD requirements, and assisting each Participant/Designee/COS with the implementation of their plan/budget to ensure compliance with all applicable SD rules and regulations. The role also includes writing Staff Action Plans and uploading them into the electronic system if needed, monitoring monthly budget expenditures to ensure the Participant does not exceed the approved SD budget, and attending mandatory annual and semi-annual Life Plan meetings and completing required notes for the file. Additionally, the Case Manager will support individuals with disabilities to utilize their services and funds to live in the community. Communication will primarily be via phone, email, or zoom with the individuals and their support team, assisting them to utilize their budget according to OPWDD rules and regulations. The role requires obtaining and maintaining required documentation, and reviewing all documentation for accuracy and guidelines adherence. The Case Manager must effectively perform assigned duties in a remote work environment while maintaining productivity, responsiveness, quality standards, and compliance with Agency policies and procedures. All communications must be documented, and confidentiality of all work-related information must be maintained.

Requirements

  • High School diploma or equivalent required.
  • Two years’ customer service in a fast-paced work environment with one year’s experience working in an environment requiring strong attention to detail and the ability to make some decisions independently.
  • Strong customer service and computer skills.
  • Must be proficient in Microsoft Office suite applications for preparation of documents, spreadsheets and data management using Excel.
  • Sensitivity and awareness of issues related to cultural diversity.
  • Strong computer skills are required.
  • Strong verbal and written communication, attention to detail, problem solving, excellent organization, time management, and ability to work with a diverse population of individuals is required.
  • Must be able to work independently and initiate work in a fast-paced environment.
  • Excellent Customer Service skills required.
  • Must demonstrate the ability to effectively prioritize workload, manage competing deadlines, work independently, and maintain productivity in a remote work environment.
  • Must be flexible when needed to accommodate meetings outside regular business hours.
  • Maintain a private workspace to protect PHI.
  • Be available to report to the office when required for meetings, training, or other business needs.
  • Maintain a reliable internet connection and notify supervisor of any internet issues immediately.
  • Be accessible to your supervisor for work related communication during working hours.
  • Maintain regular and reliable attendance and actively participate in required meetings, trainings, supervision sessions, and Agency activities.
  • Maintain a professional work environment suitable for conducting confidential participant, employee, and Agency business.

Nice To Haves

  • College degree in Human Services or related field preferred.
  • At least one year’s experience in a human services organization is preferred.
  • Basic knowledge of human services, OPWDD regulations, Medicaid, disability awareness, understanding of the Independent Living philosophy, and case management is preferred.

Responsibilities
