About The Position

Alcohol and Drug Freedom Center of Knox County is seeking a team-focused Case Manager to work full-time with clients. The Case Manager provides coordination and referrals for clients who need ancillary services to help them achieve their individual treatment plans. This includes assisting clients with housing, medical care, food, employment, and transportation, in addition to working with other community organizations to provide continuity of care through collaborative services and information sharing. The position is focused on meeting and assessing the needs of clients upon referral and at regular intervals, maintaining data for client records and funding sources as needed to verify applicable and successful service delivery, and working with primary counselors to develop the best plan for care of the clients and monitoring implementation of the treatment plan and client progress. This is an office and community-based position that involves some travel and transporting of clients within the county.

Requirements

  • High school diploma or equivalent
  • Valid Ohio Driver License
  • Two years of driving experience
  • Maintain and show proof of full automobile insurance coverage
  • Ability to pass a BCI check
  • Ability to pass a post offer/pre-employment drug screen

Nice To Haves

  • CDCA (Chemical Dependency Counselor Assistant) credential preferred (expected to be obtained within one year of employment if not already obtained)
