The Case Manager helps clients get the support they need to access a wide range of resources. Case Managers provide personalized support and guidance to access HopeSource programs as well as provide a direct connection to vetted community agencies and resources that support the needs of the individual. Supports include but are not limited to addressing economic barriers, providing basic needs (including, but not limited to, housing, food, transportation, utility payments, and material goods), and other supportive services such as workforce development, health and mental health care, recovery, and life skills education. Case Managers are skilled, effective communicators who have a problem solving mindset and are responsible for ongoing community resource knowledge. Case Managers collaborate extensively within the community to overcome logistical barriers and increase care quality, improve care continuity, and improve the customer experience.
