About The Position
Rite of Passage Team is hiring for a Case Manager at Palm Beach Youth Academy in West Palm Beach, Florida. The academy provides a healing environment for at-risk males, offering evidence-based, mental health, and substance abuse services designed to support recovery and improve overall well-being. The Case Manager will have the unique opportunity to create a positive, safe, and supportive environment for the youth served while building a career rich in growth opportunities and self-fulfillment.
Requirements
- Possess a BA/BS in a related field.
- Have prior case management experience, including three years' experience with juveniles.
- Be at least 21 years of age.
- Be able to pass a criminal background check, drug screen (THC is not tested for pre-employment), and TB test.
- Be able to pass a search of the child abuse central registry.
Responsibilities
- Administering, developing, and facilitating the completion of case plans and treatment programs in accordance with the requests of the placing agency and the needs of each student on their case load.
- Collaborating with other professional staff to establish and meet the goals of each youth's treatment plan by providing individual counseling and skill development.
- Communicating with families, probation officers, referral agencies, etc., on the progress of each youth on your case load.
- Providing an atmosphere that is supportive of the youth’s needs.
- Monitoring and documenting clinical behaviors and activities.
- Participating and assisting with educational, social, athletic, and recreational activities.
