About The Position

The Substance Use Disorder Case Manager’s primary role, under the direction of the Regional and Clinical Leadership, is to provide support and coordinate services based on the clients’ needs and treatment plans. The case manager will actively engage in a professional, collaborative process to support our clients’ substance use disorder and physical health needs. As a case manager, you will assess, plan, implement, coordinate, monitor, advocate, and evaluate the options and services required to promote recovery, health, quality, and cost-effective outcomes for our clients, by ensuring client safety and adherence to quality care standards. In this role, you will work closely with other care team members to plan, link, advocate, coordinate and monitor client care. This collaboration will help clients access a wide range of resources, including but not limited to health services, financial services, housing, employment, education, sober support systems and social services. The case manager is also responsible for successfully managing their caseload and communicating with the treatment team as it relates to case management across various disciplines.

Requirements

  • Candidate must have a minimum of 1 year’s experience working with the Substance Use Disorder population.
  • This position requires the ability to work under stressful conditions and to work irregular hours.
  • Requires sitting for extended periods of time, viewing computer monitors, and keyboarding.
  • Candidates must possess excellent verbal and written communication skills to communicate professionally via telephone and in writing.
  • Strong grammar and writing skills are crucial for clear and concise communication with clients, colleagues, and other healthcare professionals.
  • Candidates must achieve competency in the Electronic Health Record (HER) program, telemedicine services, and any additional software programs utilized by the company, ensuring efficient access and updating of client information.
  • The role requires fine dexterity, adept handling, and proficient gripping abilities to perform various tasks.
  • The candidate must pass all federal and state background checks, and their educational and work history will be confirmed in compliance with company policies.

  • Master’s or Bachelor’s degree preferred
  • Pursuing a CDCA is highly recommended.

Responsibilities

  • Act as a liaison between the therapist, other care team members, client, and client’s family to coordinate the best care.
  • Arrange family visits, coordinate transportation, and ensure clients’needs are met.
  • Maintain clear and ongoing communication between the family, referral source, and Path Behavioral Healthcare.
  • Attend court hearings asappropriate.
  • Ensure documentation and billing adhere to policy, Path Behavioral Healthcare standards of practice, and all applicable compliance standards within the EHR system.
  • Maintain compliance with chart/documentation audit criteria, billing reviews, and Phase System of Care requirements.
  • Attend supervisory and team meetings; presentaccuratecase management reports reflectingservices provided and clients’ responses to treatment. If case manager holdsaCDCAtheyare required toattend weekly Clinical Supervision.
  • Ifcasemanager holds a CDCA orhigherthey maybe requiredtoprovideevidence-basedgroup therapy.
  • Connect and/or attend sober support meetings with clients andassistclientin seeking a sponsor.
  • Collaborate with multidisciplinaryteamincluding medical professionals, counselors, social workers, and support staff to provide integrated care.
  • Communicate effectively (verbal and/or written)with clients’ families, legal representatives, and community partners to ensure a comprehensive and coordinated approach to care.
  • Conductrandom urine drug screening as needed/recommended byserviceteam.
  • Uphold confidentiality and ethical standards in all interactions, respectingclients’rightsand privacy.
  • Oversee the implementation of the individual plan of service, supporting the client’s aspirations,goalsand desires foroptimizingindependence, promoting recovery, and developing andmaintainingnatural support systems.
  • Ensure ongoing client participation in discussions of their plans,goalsand status.
  • Inform therapist of continued use and/or lapse/relapse to assure clientsafetyandthe client is within theappropriate levelof care.
  • Coordinate andassistthe client in crisis intervention and discharge planning, including community support systemsfollowing residential and/or withdrawal management.
  • Maintain an organized practice and schedule, be at work on time and ready to work, and leave work at the completion of theclientcare responsibilities.
  • Reduce client reschedulingby informing theappropriate personof time off work.
  • Maintain strict compliance with HIPPA and 42-CFR guidelines,confidentiality agreements, billing and coding guidelines, and company policy.
  • Maintain a demeanor of positive professionalism and serve as a model for others to exemplify the mission and vision of Path Behavioral Healthcare.
  • Assess and reassessneedsof the clients as neededon a monthly basis.
  • Servicesidentifiedin the ISP.
  • Assistancewith accessing natural support systems in the community.
  • Coordination with formal/informal community services and systems.
  • SymptomMonitoring.
  • Coordination andassistancein crisis management and stabilization.
  • Coordination andassistanceinlocatingand placement of higher levels of care if needed (ie: withdrawal management, residential or sober living).
  • Client advocacy and outreach.
  • Seek education and trainingspecific to client care; provide client education
  • Behavioral Health interventions addressing symptoms, behaviors,thought processes, triggers, post-acute withdrawal symptoms,etc, to help clients eliminate barriers to seeking or maintaining recovery, education and employment.
  • Activities that increase theclient’scapacity to remain in recovery and positivelyimpacttheir environment.
  • Assumeadditionalduties asassigned by theRegional and Clinical Leadership team.
  • Maintain an active, unrestricted Ohio licensure to provide theSubstance Use Disorder Counseling Services.
  • Attends all required continuing education opportunities per the regulations of your licensing body.
