The Case Manager provides ongoing, documented case management to adults enrolled in Community Health & Recovery, a low-threshold program offering harm reduction, health promotion, care coordination, and recovery support for people who use drugs or are at risk of overdose or drug-related harm. Using participant-centered, trauma-informed, culturally responsive, and harm reduction approaches, the Case Manager conducts assessments, develops service plans, coordinates referrals, tracks progress, and helps participants work toward their goals. The role supports access to medical care, behavioral health services, substance use treatment, hepatitis care, housing, food assistance, benefits, legal services, and other health and social supports, while providing advocacy, crisis support, health promotion, and group services as appropriate. This role is distinct from brief drop-in harm reduction encounters because it maintains an ongoing, individualized case management relationship with each participant.
