About The Position

Harbor Point Behavioral Health Center offers hope and healing for youth ages 7 to 17 at the time of admission with several psychiatric diagnoses and/or developmental disabilities. Located in Portsmouth, VA, Harbor Point provides intensive, residential treatment to youth in a comfortable setting that is conducive to healing. Our facility specializes in treating children and adolescents with depression, bipolar, ADHD, and other mental health issues. We also have specific treatment programs for those with intellectual disabilities or those with self-injurious behavior. Harbor Point and its programs are fully accredited by The Joint Commission and the Virginia Association of Independent Specialized Educational Facilities. We are licensed by The Virginia Department of Behavioral Health and Developmental Services and the Virginia Department of Education. We are a TRICARE®-approved and certified facility. Website: https://harborpointbhc.com/ The Case Manager provides services for youth, adolescents and their families that promote coordinated, effective treatment by facilitating the communication between the referral sources, the physician/treatment team, and payer, and by managing timely transitions for the individual throughout their stay in the residential program.

Requirements

  • Bacherlor's Degree in appropriate Human Services field.
  • Must have minimum 1 year of experience working with adolescents in a psychiatric or substance abuse facility.
  • While performing the duties of this job, the employee is regularly required to stand, sit, walk, climb or balance and stoop, kneel, talk, and hear; use hands to finger, handle or feel; and reach with hands and arms.
  • The employee must regularly lift and/or move up to 10 pounds and occasionally lift and/or move up to 50 pounds.
  • Specific vision abilities required by this job include close, distant, and peripheral vision, color and depth perception, and ability to adjust focus.

Responsibilities

  • Conducts family engagement sessions weekly with residents who have family therapy with the therapists.
  • Establish and maintain contact twice a month with the assigned resident’s guardians, probation officers, assignee case managers, GALs, etc.
  • Complete all documentation including, but not limited to, biopsychosocial assessments, obtaining initial ITP signature page, ISPs, case management notes for family engagement, and communication with parents/guardians, setting up discharge summaries, etc.
  • Attend scheduled meetings including, but not limited to, clinical flash, clinical meeting, FAPT (when needed), court (when needed), treatment team (when needed), etc.
  • Establish and maintain clinical data.
  • Participates in implementation of clinical program.
  • Performs any additional duties assigned from management.
