Screens and assesses the eligibility of elderly candidates for participation in the Homebound Meal Program. Educates the community on various homebound meal outreach programs and the importance of utilizing the program. Distributes educational materials in the community. Visits homes to determine client needs, develops a plan to meet needs. Identifies and monitors clients for homebound meal program. Completes required paperwork in compliance with state and program regulations. Creates and maintains records in SERV Tracker software. Administers initial nutritional screening in the form of a questionnaire. Helps client obtain necessary resources. Refers clients with social and emotional problems to other community agencies for assistance. Refers clients to the Director of Nutrition Services for nutrition counseling as needed. Documents client cases, including assessments and reassessments, and maintains records on each client. Prepares reports as required by various federal, state, and local programs. Helps organize and participate in fundraisers benefiting the program.
