About The Position

At RHD, we believe every person deserves access to compassionate, person-centered services that support recovery, independence, and meaningful participation in the community. We are seeking a dedicated Case Manager who wants to make a positive difference in the lives of individuals receiving behavioral health services. In this role, you will work collaboratively with program participants, clinical and medical professionals, community providers, and other stakeholders to coordinate services and support individualized recovery goals. Requirements

  • Bachelor’s degree in human services or a related field.
  • At least two years of general work experience providing support to individuals receiving behavioral health, addiction recovery, or behavioral health home services.
  • Intermediate computer skills.
  • Valid driver’s license.
  • Ability to travel throughout the program’s service-delivery area.
  • Ability to transport program participants to appointments as required.

Nice To Haves

  • At least one year of case management experience.

Responsibilities

  • Participate as a member of a multidisciplinary team providing holistic, values-based services.
  • Assist with the development and review of individualized recovery plans in collaboration with the program participant, clinical and medical staff, and other stakeholders identified with the participant.
  • Help determine the resources and services needed to support each participant’s individualized goals.
  • Advocate for program participants to help ensure access to appropriate services and supports.
  • Identify program needs and service gaps and recommend strategies for addressing them.
  • Develop and maintain productive relationships with community agencies.
  • Connect program participants with available community resources and services.
  • Provide community-based services to program participants.
  • Educate participants about available services, supports, and resources.
  • Assist participants with making meaningful connections in their communities.
  • Support participants in working toward the goals identified in their recovery plans.
  • Transport program participants to appointments as required.
  • Complete assessments, service documentation, and other records accurately and within required timeframes.
  • Maintain documentation in accordance with RHD requirements and applicable funder, licensing, and accreditation standards.
  • Participate in program planning, coordination, and service-review activities.
  • Perform other job-related duties as assigned.
