The Case Manager is responsible for managing all aspects of individual caseloads and case records for residents assigned to them. This role involves teaching classes, providing discharge planning, assessing resident needs, performing intake procedures, applying behavior management systems, and conducting crisis intervention. The Case Manager will also prepare written reports, attend meetings, coordinate counseling services, and maintain relationships with treatment facilities and the public. Additionally, they may make court appearances, maintain confidentiality, conduct home/employment visits, and transport residents to community resources. This position may also be called upon to supplement facility coverage during emergencies.
