About The Position
Baptist Health is seeking a dedicated and compassionate Registered Nurse to join our Case Management team. This is a Full Time day shift opportunity and plays a key role in supporting patients throughout their hospital stay by ensuring coordinated, high-quality care.
Requirements
- 1 year case management experience
- Associate of Science in Nursing required
- Basic Life Support (BLS) required
- FL Licensed Registered Nurse required
Nice To Haves
- Bachelor of Science in Nursing preferred
Responsibilities
- Provide comprehensive care coordination for hospitalized patients.
- Conduct high-risk screenings and assessments to identify patient needs.
- Arrange and document post-acute care services, ensuring smooth transitions of care.
- Collaborate with a multidisciplinary healthcare team to support optimal patient outcomes.
