About The Position

The Case Manager is responsible for providing documented individual and group clinical services and case management to clients with mental health, substance abuse, or dual diagnosis problems. This role involves developing, implementing, and revising individual treatment plans for assigned clients within established time frames with client participation. The Case Manager will also refer clients to appropriate community services as necessary and participate in multidisciplinary team treatment planning, case review, and case conferences.

Requirements

  • At least one year experience working in a substance abuse treatment, case management role or closely related field
  • Minimum of a high school diploma

Nice To Haves

  • LADC I or CADAC certification preferred, but not required
  • Bachelor’s Degree Preferred

Responsibilities

  • Referring clients to appropriate services in the community as necessary.
