About The Position

As the Ridge Case Manager, you will collaborate with other members of the clinical team, to serve as a liaison for the treatment team, client, families and aftercare services.

Requirements

  • Minimum 2 years of experience in a residential or therapeutic setting.
  • Leadership skills that foster a culture of inclusivity and excellence.
  • Excellent communication, leadership, and organizational skills.
  • Strong crisis management and conflict resolution skills.

Nice To Haves

  • Bachelor's degree in social work, psychology, healthcare administration, or a related field (preferred).

Responsibilities

  • Provide comprehensive case management, including assessment, care planning, and ongoing support.
  • Participate in the interdisciplinary treatment planning process and perform proactive discharge planning.
  • Provide education to clients and their families regarding community resources, benefits, and coverage.
  • Coordinate and facilitate meetings between the clients/families/residential care team. You will assist clients and families in making difficult decisions while honoring their preferences and values.
  • Facilitate group sessions and support recovery-focused programming.
  • Ability to successfully document all clinical contacts and documentation through Electronic Medical System.
  • Support a safe, structured, and trauma-informed residential environment.
  • Facilitate and provide timely documentation.
  • Adhering to company policies and procedures regarding confidentiality and privacy.
  • Other duties may be assigned.
