About The Position

The Case Manager will utilize appropriate therapeutic models to aid clients in understanding and managing their symptoms in a variety of environments. This role involves conducting program-specific assessments and screens, providing case management, and working collaboratively with clients' natural supports and treatment teams. The Case Manager will connect clients to prosocial activities, provide crisis intervention and de-escalation as needed, and demonstrate competency in age-related or specialty issues and developmental needs for the population served. Additionally, the role requires demonstrating the ability to teach and support the learning of a broad range of skills, including emotion regulation, age-appropriate activities, and competencies for living in a home-like setting. The Case Manager will plan and participate in activities to support and increase skills and competencies, document all services and contacts in compliance with agency standards, and transport clients as required by the program. Participation in staff meetings, supervision, training, and other assigned meetings is also expected, along with performing additional duties as required.

Requirements

  • Associate’s required plus bachelor’s equivalent experience (2 years)
  • At least two (2) years’ experience in a mental health setting preferred.
  • Valid driver’s license.

  • Plan and actively participate in activities to support and increase skills and competencies.
  • Documents all services and contacts with clients, collateral contacts and appointments in compliance with agency standards.
  • Transports clients as required by program.
  • Participates in staff meetings, supervision, training and other meetings as assigned.
  • Additional duties as required.
