About The Position

SSM Health Physical Therapy’s Day Institute in Olivette, Missouri, is seeking a full-time Case Manager for its specialized outpatient rehabilitation program. The role may require floating within the region to assist other Day Institute facilities. The program serves people recovering from traumatic injury or illness who no longer need 24-hour nursing or acute rehabilitative care. It uses a comprehensive, multidisciplinary approach to build skills and maximize independence. The position follows a Monday–Friday schedule, 8:00 a.m.–4:30 p.m. Experience in physical medicine and rehabilitation is strongly preferred. Requirements

• Professional healthcare degree in one of the following: Nursing, Social Work, or licensed PT, OT, or SLP.
• Experience in physical medicine and rehabilitation is strongly preferred.

Nice To Haves

• One year of medical rehabilitation case management experience is preferred.

Responsibilities

• Monitor and assess the patient’s plan of care from pre-admission through post-discharge under general direction.
• Assist patients and families, as part of the treatment team, in setting goals to maximize patient independence at home and in the community.
• Interact with payers to obtain necessary authorizations for initiating and continuing care throughout the program admission.
