About The Position

IPMG is seeking a full-time Case Manager for Boone County. This hybrid position requires travel to community-based meetings multiple times per week and allows for remote work from a home office setting. Applicants must reside within the State of Indiana or within 10 miles of the state line due to the requirement for in-person meetings. IPMG is 100% Employee Owned through an Employee Stock Ownership Plan (ESOP), allowing employees to share in the company's growth. The role involves working with Individuals on four Medicaid Waivers: Family Supports Waiver (FSW), Community Integrated Habilitation Waiver (CIH), Health & Wellness Waiver (H&W), and Traumatic Brain Injury Waiver (TBI).

Requirements

A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, OR a Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
A valid driver's license, car insurance, and reliable transportation
The ability to pass thorough background screens
Designated home office setting with computer, internet service, and smart phone

Responsibilities

Provide information and education to help Individuals understand the Medicaid waiver process
Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals served
Help Individuals access services and supports they need to meet their goals
Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed
