About The Position

IPMG is offering a $500 sign-on bonus within the first 6 months of employment. IPMG is 100% Employee Owned through an Employee Stock Ownership Plan (ESOP), allowing employees to share in the company's growth. This is a full-time, hybrid position requiring travel to community-based meetings multiple times per week and working remotely from a home office. Applicants must live within the State of Indiana or within 10 miles of the state line due to in-person meeting requirements.

Requirements

  • A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, OR a Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
  • A valid driver's license, car insurance, and reliable transportation
  • The ability to pass thorough background screens
  • Designated home office setting with computer, internet service, and smart phone

Nice To Haves

  • Case managers work with Individuals on four Medicaid Waivers; Family Supports Waiver (FSW), Community Integrated Habilitation Waiver (CIH), Health & Wellness Waiver (H&W), and Traumatic Brain Injury Waiver (TBI)

Responsibilities

  • Provide information and education to help Individuals understand the Medicaid waiver process
  • Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals served
  • Help Individuals access services and supports they need to meet their goals
  • Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed
