About The Position

The Case Manager is a member of a patient’s care team and assists in planning, developing, and coordinating patient care in accordance with applicable federal, state, and local standards, guidelines, and regulations. The role helps ensure that the highest degree of quality care is maintained. The Case Manager works with patients, families, administration, medical and nursing staff, other departments, and community agencies to coordinate care and support smooth transitions after discharge.

Requirements

• Active LPN or RN license.
• Active fingerprint card.
• Ability to make independent decisions, follow instructions, and accept constructive criticism.
• Leadership ability and willingness to work harmoniously with other personnel.
• Good judgment and ability to maintain highly confidential information.
• Knowledge of HIPAA compliance and requirements.
• Ability to plan, organize, develop, implement, and interpret departmental programs, goals, and objectives as they relate to the Company.
• Must pass a pre-employment background check and drug screen.

Nice To Haves

• A minimum of one year of case management experience is preferred.

Responsibilities

• Meet with administration, medical and nursing staff, and other related departments to plan services, including services for patients and families.
• Partner with patients, family members, the patient’s care team, and social, health, and community agencies to support smooth transitions upon discharge from the Center.
• Ensure documentation is informative, descriptive, and consistent with the Center’s policies and procedures.
• Assist with the admission and discharge process.
• Attend care plan meetings and provide insight and professional guidance regarding patients’ needs.
