About The Position

The Case Manager reports to and works closely with the Coordinator of Case-Management and Student Development and other administrative staff. This role also works closely with the Director, Assistant Director, and Coordinators to ensure quality care is being provided to clients. Responsibilities include participating in the on-call schedule, managing crisis situations, and coordinating and facilitating patient care through assessment, evaluation, planning, and implementation. The Case Manager will also be involved in Counseling Center outreach events, tracking events and goal progress, and scheduling meetings. This role involves working closely with Mental Health Specialists, carrying a caseload for med management and higher acuity clients, conducting walk-in clinics and intakes, and developing, monitoring, and evaluating treatment plans and progress. Communication with care team members, providing referrals, and linking students to community resources are key aspects of the job. Maintaining client records, advocating for client success, scheduling/rescheduling psychiatric appointments, managing psychiatric medication refills, and preparing weekly psychiatric provider reports are also required. The Case Manager will monitor psychiatric appointments, document and monitor student attendance, outreach to clients missing appointments, manage discharge plans, and assist in developing and carrying out workshops and support groups.
