About The Position

Helen M Simpson Rehabilitation Hospital, a partnership with UPMC and Select Medical, is seeking a Per Diem Case Manager for its Inpatient Rehabilitation Hospital. This facility is dedicated to providing exceptional and compassionate care for individuals facing medical, physical, emotional, and vocational challenges resulting from brain injuries, spinal cord injuries, neurological disorders, orthopedic issues, amputations, and multiple traumas. Part-time/Per Diem positions are eligible for 401k upon reaching 1,000 hours within their first anniversary or subsequent calendar year. Requirements

• Current Licensure per state guidelines in clinical or related discipline OR a Bachelor's or Master's in a health or human services discipline.

Nice To Haves

• Previous experience in Case Management and Discharge Planning preferred.
• CCM Certification Preferred.

Responsibilities

• Coordination of health care decisions using a systematic approach to ensure treatment plans improve quality and outcomes.
• Coordination of care across the continuum.
• Promotion of cost-effective care within the allotted time frame.
• Assurance of payments for hospital-based services meeting patient-related utilization management criteria.
• Implementation of safe and appropriate discharge plans.
• Assessment of the psychosocial needs of the patient and provision of intervention as part of the discharge planning process.
