About The Position

Southern Kentucky Rehabilitation Hospital, a joint venture with Select Medical, is seeking a Per Diem Case Manager in Bowling Green, KY. This role is crucial in coordinating health care decisions to improve patient outcomes and ensure cost-effective care. The hospital focuses on providing exceptional and compassionate care for individuals with a wide range of injuries and disorders, including brain injuries, spinal cord injuries, neurological disorders, orthopedic issues, amputations, and multiple traumas.

Requirements

• Current Licensure per state guidelines in clinical or related discipline OR a Bachelor's or Master's in a health or human services discipline.

Nice To Haves

• Previous experience in Case Management and Discharge Planning preferred.
• CCM Certification Preferred.

Responsibilities

• Coordination of health care decisions using a systematic approach to assure treatment plans that improve quality and outcomes.
• Coordination of care across the continuum.
• Promotion of cost-effective care within the allotted time frame.
• Assuring payments of hospital-based services meeting patient-related utilization management criteria.
• Implementation of safe and appropriate discharge plans.
• Assessment of the psychosocial needs of the patient and providing intervention as part of the discharge planning process.
