About The Position

BHC Alhambra Hospital is seeking a Clinical Case Manager – Per Diem to join their team. This facility, located in Rosemead, California, is a 115-bed acute psychiatric hospital serving children, adolescents, and adults, offering inpatient, residential, partial hospitalization, and intensive outpatient options. The Case Manager will manage patient treatment activities, including clinical screening of potential admissions, establishing treatment plans, interfacing with the treatment team and external organizations, and overseeing discharge planning.

Requirements

Strong written, verbal and interpersonal communication skills are required.

Nice To Haves

R.N. or Bachelor, Masters or Doctorate Degree from an accredited college or university in social work, mental health, psychology or a related field preferred.
A minimum of four (4) years direct clinical experience in a psychiatric or mental health setting preferred.
Experience in patient assessment, family motivation, treatment planning and communication with external review organizations is preferred.

Responsibilities

Participate in the implementation of the treatment plan and conduct an ongoing review of the plan to ensure effective implementation of all treatment interventions.
Maintain ongoing direct contact with the attending practitioner, treatment coordinator, and various members of the team.
Assist in ensuring that the patient attends the appropriate level of care or program as indicated on the treatment plan or as needed when there are changes in the patient’s status.
Interface with the staff of the appropriate level of care to facilitate a smooth transition at the time of transfer.
Maintain necessary documentation of all services and the quality of care.
Assure proper tracking of all reviews.
