About The Position

Merakey is seeking a Case Manager to join their team at the Riverside Wellness Village in Philadelphia, PA. This role is part of a groundbreaking program, the largest of its kind, providing safe housing and treatment opportunities for individuals in recovery. The Case Manager will work directly with adult men in the early stages of recovery from chemical or alcohol dependency, co-occurring disorders, serious mental illness, and homelessness. This is a direct care position in a transitional housing setting, reporting to the program manager or director.

Requirements

  • High school diploma or GED
  • A minimum of one (1) year of experience in social work or case management
  • Proficient in Microsoft Word, Excel, SPSS, email communication platforms, employee portals, and timekeeping systems such as Teletime.
  • Strong organizational and time management skills
  • Effective teamwork and collaboration abilities
  • Excellent problem-solving and interpersonal communication skills
  • Ability to manage stress and remain composed under pressure
  • Capable of making sound decisions in high-pressure situations
  • Skilled in de-escalating and managing challenging behaviors, including anger, fear, hostility, or violence from others
  • Ability to prioritize and manage multiple tasks simultaneously
  • Comfortable working independently and in both confined and crowded environments

Nice To Haves

  • Bachelor’s degree in social work or a related Human Services field is preferred.

Responsibilities

  • Manage the needs of program residents.
  • Helps residents living in the recovery house create goals focused on their recovery, improving relationships, obtaining an income, and securing housing.
  • Meets with residents bi-weekly and has a caseload ratio of 35:1.
  • Link residents to various community-based services and resources as needed.
  • Ensure all progress reports are completed properly and on-time.
  • Incident reports which include incident reports, reporting to immediate supervisor and following procedure as stated in Merakey policy.
  • Encourage and verify that residents with income are saving money.
  • Data input and outcome analysis.
  • Life Skills Group facilitation (i.e. money management, housing acquisition, nutrition).
  • Assist with intakes and various other tasks in an effort to support program operations and the program director.
