Requirements

  • Bachelor's degree in Psychology, Social Work, Counseling, Human Services, or a related field required.
  • Valid California Driver License required.
  • Clean driving record required.
  • Must be insurable under Alcott's auto liability insurance policy.
  • Live Scan clearance required.
  • Evidence of a clear criminal record and eligibility to claim Medi-Cal.

Nice To Haves

  • Experience working with individuals living with serious mental illness preferred.
  • Experience with Los Angeles County Department of Mental Health documentation preferred.
  • Experience in community mental health, supportive housing, case management, or related social service settings preferred.
  • Bilingual English/Spanish candidates are strongly encouraged to apply.
  • Strong interpersonal, communication, and engagement skills.
  • Ability to work independently in community and field-based settings.
  • Ability to manage competing priorities and maintain productivity standards.
  • Basic computer proficiency and ability to utilize electronic health record systems.
  • Strong organizational skills and attention to detail.

Responsibilities

  • Coordinate medical follow-up appointments and services for clients with co-occurring medical and behavioral health conditions.
  • Develop and implement treatment plans in collaboration with clients and multidisciplinary team members.
  • Provide rehabilitation services, life skills training, and support for independent living.
  • Assist clients with locating, securing, and maintaining stable housing placements.
  • Connect clients to community resources and supportive services.
  • Complete accurate, thorough, and timely documentation in accordance with Los Angeles County Department of Mental Health (LACDMH) and Medi-Cal standards.
  • Maintain required productivity and service utilization expectations.
  • Ensure documentation and service delivery remain compliant with agency and regulatory requirements.
  • Respond to client crises and provide appropriate intervention and stabilization services.
  • Participate in rotating on-call responsibilities, including after-hours crisis response when assigned.
  • Conduct field-based outreach and engagement activities throughout Los Angeles County.
  • Work closely with multidisciplinary team members, service providers, and community partners.
  • Communicate effectively with clients, families, providers, and stakeholders.
  • Maintain professionalism, accountability, and client-centered service delivery.
