About The Position
The Case Manager, Community Pet Support Programs is responsible for overseeing APA's pet retention and community support programs, including supervision of the Pet Owner Support Specialist and Community Resource Specialist. The Case Manager works to ensure pet owners have access to the resources needed to keep their pets safe in their homes while advancing APA's mission through strategic partnerships, effective service delivery, and community outreach.
Requirements
- Bachelor's Degree in Social Work (BSW) or equivalent social service degree required
- Minimum of 3 years of case management, social services, community outreach, or related experience.
- Valid driver's license and ability to travel locally.
Nice To Haves
- Master of Social Work (MSW) preferred.
- Prior supervisory experience preferred
- Strong communication, organizational, and relationship-building skills.
Responsibilities
- Supervise and support the Pet Owner Support Specialist and Community Resource Specialist.
- Provide coaching, case consultation, performance feedback, and program oversight.
- Assist with planning, implementation, and evaluation of community support programs.
- Maintain a caseload of up to 10 active Safe Care cases.
- Conduct assessments, develop service plans, and coordinate resources for clients in crisis.
- Collaborate with internal teams, veterinary partners, and community organizations to support successful outcomes.
- Develop and maintain partnerships with social service agencies, veterinary providers, community organizations, and resource partners.
- Identify and secure new pet food resources and donation opportunities.
- Represent APA at community meetings and outreach events.
- Recruit, train, and support volunteers and foster caregivers to assist with community programs and events.
- Ensure adequate volunteer and foster support for Safe Care, outreach initiatives, vaccine clinics, and pet food distribution.
- Monitor program outcomes and maintain accurate case documentation.
- Assist with grant reporting, data collection, and program evaluation.
- Assist with identify opportunities for program improvement and growth.
- Maintain confidentiality and professionalism.
- Perform other duties as assigned.
