About The Position

At Resources for Human Development (RHD), we believe every family deserves safe housing, stability, and the opportunity to thrive. We are seeking a compassionate and dedicated Case Manager to join the team at Woodstock Family Center, an emergency housing program serving single parents, couples, and families with children experiencing homelessness. As a Case Manager, you will play a critical role in helping families identify barriers, access resources, achieve housing stability, and build pathways toward long-term success. This position is ideal for professionals who are passionate about advocacy, community engagement, and empowering families to reach their goals.

Requirements

  • Bachelor's Degree in Human Services or a related field.
  • Minimum of two years of experience providing support to individuals and families in behavioral health, addiction recovery, housing, homeless services, intellectual and developmental disabilities, or related human services programs.
  • Valid Driver's License and acceptable driving record.
  • Intermediate computer skills.
  • Strong verbal and written communication skills.
  • Ability to work effectively in community-based environments.
  • Ability to work collaboratively while maintaining independent case management responsibilities.

Nice To Haves

  • One year of Case Management experience.
  • Experience working with individuals and families experiencing homelessness.
  • Knowledge of community housing resources and social services.
  • Experience within behavioral health, recovery, or family-focused programs.

Responsibilities

  • Serve as part of a multidisciplinary team providing holistic, person-centered services.
  • Assist residents in developing individualized recovery, housing, and service plans.
  • Support families in identifying goals and overcoming barriers to stability.
  • Connect residents to housing resources, employment supports, healthcare providers, educational services, and community programs.
  • Maintain relationships with community agencies and referral partners.
  • Advocate for residents to ensure access to necessary services and supports.
  • Provide community-based case management and resource coordination.
  • Monitor progress toward service plan goals and assist with plan updates.
  • Complete assessments, case notes, reports, and required documentation in accordance with organizational, licensing, and funding requirements.
  • Participate in identifying service gaps and recommending strategies to improve outcomes.
  • Ensure access to intensive case management services as required by program standards.
  • Perform additional duties as assigned.
