Requirements

  • Bachelor's degree in social sciences and related fields or relevant experience
  • At least one year of experience providing intensive case management as defined by long term service delivery, weekly meetings with clients on case load
  • Experience engaging in assessment and service planning for clients
  • Experience with documentation of client progress
  • Experience with Trauma informed care

  • Navigation of mass housing assistance programs like RAFT
  • Ability to navigate multiple flights of stairs regularly to access client homes and buildings without elevator access

Responsibilities

  • Provide individualized case management from a whole family perspective.
  • Client engagement includes assessment, service planning, implementation, and progress monitoring.
  • Service delivery will follow an individualized service plan that will be created with clients.
  • Work with residents and clients experiencing complex challenges such as rental arrears, income instability, benefits loss, mental and physical health conditions, and/or other barriers to stabilization.
  • Provide support across programs within JPNDC.
  • Coordinate and engage in transition meetings with other departments.
  • Be available to assist with isolated short term needs for residents in addition to longer term clients.
  • Work from multiple job sites and meet participants in their homes, if needed.
  • Attend Housing Court when needed as assessed by program supervisor.
  • Maintain strong working knowledge of community, city, and state resources and connect clients directly to services including employment, education, legal assistance, housing support, childcare, financial benefits, and emergency resources.
  • Serve as a liaison with external service providers and collaborate on case planning, referrals, and follow-up.
  • Encourage program participation through various outreach methods including but not limited to flyering, presentations, events, and networking.
  • Coordinate community-building events with residents.
  • Develop materials to market events and provide resources to clients.
  • Provide occasional group workshops.
  • Keep ongoing record of interactions in a timely and professional manner.
  • Track changes and progress through case planning and assessment data.
  • Process referrals through multiple domains.
  • Uphold confidentiality, professionalism, trauma-informed, and culturally responsive practice at all times.
  • Perform clerical tasks such as photocopying, scanning, filing, faxing, and printing.
  • Participate in required training and professional development.
  • Perform related duties as assigned by supervisor.
