About The Position

The Case Manager is responsible for providing case management services to consumers of the Continuing Day Treatment Program. They work closely with the Director to ensure that the agency’s mission of providing individual counseling, group therapy and crisis intervention, for SPMI persons within a drug-and violence-free Treatment Program. They are responsible for adhering to the Office of Mental Health and Argus’ policies and procedures, and for bringing all significant concerns regarding the program and/or clients to the Director and the Clinical Team.

Requirements

  • Must be QHP or equivalent, as assessed by Argus Community Inc. and Executive Staff.
  • Must have minimum of three years’ experience working with SPMI populations, preferably in a Continuing Day Treatment Program or similar environment.
  • Must have working knowledge of the regulations that govern SPMI/Day Treatment Programs.
  • Must be computer literate and have excellent interpersonal, written and verbal communication skills.
  • We follow the NYC Workplace Vaccination requirements.

Responsibilities

  • Facilitates therapeutic groups and seminars as assigned, documents results in group notes and significant issues in progress notes.
  • Conducts weekly caseload groups where treatment plan goals are addressed, enters results in progress notes.
  • Conducts individuals counseling sessions at least once every two weeks or per OMH/Medicaid regulations with each person on caseload, documents results in clinical folder. The content of counseling sessions should relate to treatment plan goals and must meet OMH regulations.
  • Maintains timely clinical documentation at all times. Clinical documentation includes, but is not limited to, progress notes, treatment plans, monthly psychiatric evaluations, toxicology reports, medical record/PPDs, URs and billing sheets.
  • Ensures that all incidents that occur during program hours are documented and reviewed by the Director and submitted to the Compliance Department and Office of Mental Health in a timely manner, when appropriate.
  • Monitors and ensures the clients’ personal hygiene and ADL skills are developed in a consistent and satisfactory manner; issues must be addressed in groups and individually.
  • Observes and relates to consumers on a daily basis, providing feedback to the treatment team regarding behaviors and problems; such matters must be noted in the daily communication log.
  • Attends weekly progress review meetings, staff meetings, treatment plans, case conferences and trainings.
  • Work closely with Psychiatrist, Nurse Practitioner in Psychiatry (NPP), and coordinates care among collaterals, mandate agencies, and residential treatment program.
  • Participates in the identification of health and legal problems of the consumers and communicates, in a timely manner, such information to the treatment team.
