We invite you to partner with us in our mission to improve mental healthcare. This role serves adults, 18 plus, through outpatient community-based services and support throughout Fresno County. The Case Manager provides services to clients in the office, community, and their homes, and may be required to transport clients/client family members utilizing a personal vehicle.

Requirements

An associate degree in a social service field plus two (2) years of experience in a mental/behavioral health setting.
A valid California Driver’s License and eligibility to meet agency driving criteria.

Nice To Haves

A bachelor’s degree in a social science field plus six (6) months of experience (preferred).
Relevant internship experience may be substituted for a portion of the required work history.

Responsibilities

Provides services to clients in the office, community, and their homes.
Travels within the community and may transport clients using a personal vehicle.
Works with clients and families to provide case management services and support their needs.
Participates in treatment team meetings to help ensure client goals and recommendations are followed.
Maintains accurate and timely documentation in the Electronic Medical Record system.
Assists with client and family assessments to identify strengths and needs, monitors progress toward treatment goals, and communicates updates to the primary therapist.
