About The Position

IPMG is seeking a full-time Case Manager for Vigo County. This hybrid position requires travel to community-based meetings multiple times per week and allows for remote work from a home office setting. As an employee-owned company, IPMG offers a unique opportunity to share in the company's growth through an Employee Stock Ownership Plan (ESOP). The role involves providing information and education on the Medicaid waiver process, creating individualized support plans, assisting individuals in accessing necessary services, and facilitating community-based team meetings.

Requirements

A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or A Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
A valid driver's license, car insurance, and reliable transportation
The ability to pass thorough background screens
Designated home office setting with computer, internet service, and smart phone

Responsibilities

Provide information and education to help Individuals understand the Medicaid waiver process
Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
Help Individuals access services and supports they need to meet their goals
Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed
