The Case Manager provides comprehensive, trauma informed, and culturally responsive supportive services to families residing in the Innovative Urban Village supportive housing program. Case Managers conduct assessments, develop individualized service plans, provide counseling and crisis intervention, and coordinate referrals to promote long term housing stability, safety, and independence. This role requires strong engagement skills, the ability to support families impacted by domestic violence, and a commitment to evidence informed practices. Case Managers collaborate closely with Housing Specialists, the Economic Empowerment Specialist, and the Director of Clinical Services to ensure integrated, high quality service delivery aligned with ESSHI and OTDA standards.
