About The Position

The Aging Waiver Case Manager works within the Medicaid Aging Waiver program, an in-home case management service designed to support qualified seniors aged 65 or older who are on Medicaid. The primary goal is to prevent premature institutionalization by arranging essential services such as personal care, homemaking, adult day care, respite care, chore services, meals, adult companion, non-medical transportation, and medical equipment. This role also involves assisting individuals in navigating the Medicare system through one-on-one counseling and outreach events, including submitting Medicaid applications, and educating seniors about healthcare fraud and abuse. Extensive training will be provided for both programs.

Requirements

  • SSW or related license.
  • Bachelor's degree in social work or related field.
  • SSW or greater license.
  • Valid Utah driver's license.
  • Basic computer skills.

Nice To Haves

  • At least one year working with elderly population or younger people with disabilities.
  • Can speak Spanish.

Responsibilities

  • Arrange services for qualified seniors age 65 or older and on Medicaid to avoid premature institutionalization.

  • Visit each client each month.
  • Keep up with documentation.
  • Bill Medicaid.
