The Aging Waiver Case Manager role focuses on providing in-home case management services for qualified seniors aged 65 or older who are enrolled in Medicaid. The primary goal is to prevent premature institutionalization by arranging essential services such as personal care, homemaking, adult day care, respite care, chore services, meals, adult companion, non-medical transportation, and medical equipment. Responsibilities include monthly client visits, service arrangement, meticulous documentation, and billing Medicaid. This position also involves assisting individuals in navigating the Medicare system through the State Health Insurance Information and Assistance Program (SHIP) and the Senior Medicare Patrol (SMP), which includes one-on-one counseling, outreach events, submitting Medicaid applications, and educating seniors about healthcare fraud and abuse. Extensive training will be provided for both programs.
