Case Manager - Temporary, Part-time
About The Position
As a Case Manager-RN you are responsible for facilitating resident admissions and discharges, ensuring seamless transitions through their journey. You will work with families, residents as well as internal/external partners to ensure residents receive high quality care and services. Requirements
- Current state licensure as Registered Nurse
- Be able to read, write, and communicate in English

- Previous admission/discharge experience preferred
Responsibilities
- Facilitating resident admissions and discharges, ensuring seamless transitions through their journey.
- Working with families, residents as well as internal/external partners to ensure residents receive high quality care and services.
- Conducts pre-admission assessment of prospective residents.
- Develops and ensures continuity of dialogue with hospitals/social workers in community and outlying services regarding availability within community.
