Hospitalist Case Managers are a direct extension of the Hospitalist providers who care for adult patients age 18 and older. They use nursing expertise to collaborate proactively with the interdisciplinary team on patient and family needs, necessary services across the continuum of care, and discharge transitions. Each Case Manager is accountable for a panel of patients in collaboration with a Hospitalist during hospitalization, communicates directly with Hospitalist providers about assigned patients, and carries out tasks related to each patient's care plan. They prepare a hospitalization care summary for the physician's final signature at discharge. The role coordinates care and discharge planning, supports patient transitions to the next level of care, and works with community partners and accepting facilities, primary care providers, and subspecialty offices. Case Managers identify care-plan barriers and improvement opportunities, help primary nursing staff think critically about acute medical management, and ensure discharge documentation and referrals are completed. The position is patient-facing and offers hybrid remote work capability, subject to Stormont Vail's remote-work requirements.
