About The Position

Coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge. Acts as a liaison between the patient and family and healthcare personnel to ensure necessary care is provided promptly and effectively. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks and requires a certain degree of creativity and latitude. Typically reports to a head of a department or unit.

Requirements

• Must be a registered nurse.
• At least 2 years of clinical experience.
• Familiarity with standard concepts, practices, and procedures within the field.
• Associate degree required.

Nice To Haves

• Bachelor’s degree preferred.
