Requirements
- Active RN license
- Associate Degree in Nursing or Nursing Diploma
- Minimum 2 years of recent acute care Case Management RN experience within the last year
- Strong knowledge of discharge planning, utilization management, and care coordination
Nice To Haves
- BSN
- CCM or ACM certification
- Experience in trauma, cardiac, rehabilitation, or medical-surgical settings
Responsibilities
- Coordinate discharge planning and transitions of care for hospitalized patients
- Complete psychosocial assessments and discharge planning evaluations
- Develop individualized care plans based on patient needs and goals
- Educate patients, families, and caregivers regarding treatment plans and available resources
- Collaborate with physicians, nurses, therapists, and interdisciplinary teams
- Facilitate family meetings and care conferences
- Identify barriers to discharge and arrange appropriate post-acute services
- Ensure accurate documentation and regulatory compliance
- Support quality outcomes, patient satisfaction, and efficient resource utilization
