About The Position

The Case Manager has clinical expertise and knowledge of medically appropriate criteria, applying principles of utilization and quality management, discharge planning, and clinical and financial resource management as a facilitator and active participant of the multidisciplinary patient care team. The Case Manager predicts, facilitates, and evaluates patient transitions across the care continuum and links people to systems that provide resources, services, and opportunities to achieve and maintain maximum functioning.

Requirements

• Associate’s Degree in Nursing, with ongoing efforts to obtain a Bachelor’s Degree.
• At least 3 years of clinical experience.
• Currently licensed as an RN in the State of Florida.
• Communication and interpersonal skills.
• Basic computer, analytical, grammar, and spelling skills.
• Ability to effectively multitask and prioritize responsibilities.

Nice To Haves

• Previous experience in discharge planning and utilization management.
• Knowledge of community resources.
• Assessment skills and knowledge of clinical and social factors that affect a patient’s functional status at discharge.
• Case Management certification in ACM, CCM, NACCM, or CMCN.
