Case Manager - RN
About The Position
The RN Case Manager assesses, plans, implements, and monitors a comprehensive plan of care for an assigned group of patients in collaboration with members of the medical team, including physicians, social workers, PT/OT staff, pharmacists, respiratory staff, dietary staff, and nursing. The role focuses on patients’ progression of care throughout hospitalization, coordinating with the care team on appropriate resource and service utilization and supporting a safe, effective, and efficient transition or discharge from the hospital. The stated shift hours are 8:00 AM to 6:30 PM, with days listed as Sunday through Saturday.
Requirements
- Valid RN license in the State of Florida.
- Two years of recent RN experience in a clinical capacity applicable to the designated population served.
- Case Managers hired after January 1, 2018 who do not have a BSN and/or MSN must obtain at least a BSN within three years of employment as a Case Manager.
- Case Managers hired after January 1, 2018 must obtain specialty certification within two years of employment as a Case Manager.
Nice To Haves
- BSN preferred.
- Prior Case Management experience preferred.
- Professional nursing board certification in the specialty area preferred.
