About The Position

This role involves coordinating with other departments to provide the maximum level of services. The Case Manager must demonstrate the knowledge and skill necessary to provide care, based on physical, psychosocial, educational, and safety-related criteria, appropriate to the age of the patients served in their assigned service area. These skills and knowledge can be gained through education, training, and/or experience. Other duties may be assigned.

Requirements

• Minimum of two years clinical experience in a hospital setting for Case Management required.
• Maintain current professional Texas RN licensure.
• Ability to work with multidisciplinary staff.
• Working knowledge of current Medicaid / Medicare guidelines.
• Graduate from an accredited School of Registered Nursing.

Nice To Haves

• Prefer Utilization Review / Discharge Planning background.
