About The Position

At Bethesda House, the goal is to end the cycle of poverty and homelessness by addressing individuals on a holistic level, ensuring they feel heard, respected, and safe. The organization offers hands-on help for chronic homelessness, mental health, substance abuse, disability, and systematic poverty through innovative solutions and proactive services in Schenectady County. This includes integrated primary care and behavioral health treatment programs, and assistance with financial and educational resources. The dedicated Case Management team provides triage and crisis case management to single adults needing housing or eviction prevention, assisting them in obtaining secure and sustainable housing solutions and serving as the point of access to permanent housing. The Custodial Accounts/Rep Payee Case Manager specifically manages SSI/SSD benefits for program participants, ensuring essential bills are paid while educating clients on financial management, banking, and budgeting to foster financial independence. The Residential Support Program (RSP) Case Manager oversees all aspects of the RSP subsidy program, including intake/assessment, crisis intervention, homelessness prevention, rapid re-housing, outreach, referral, and information services.

Requirements

  • Bachelor’s degree (or pursuing a bachelor’s degree) in human services or related field
  • Experience providing services to the homeless or related human services necessary
  • Knowledge of agencies, programs, and services in the community
  • Demonstrated ability to effectively engage and support individuals experiencing homelessness, mental illness, substance use challenges, and those who may be difficult to engage
  • Valid driver’s license (for one + years), ability to safely transport clients as needed to support access to services and housing stability
  • Strong computer proficiency, including experience with data entry, tracking systems, and working with financial or program-related data
  • Ability to manage a full-time schedule in a fast-paced environment while maintaining accuracy, organization, and attention to detail
  • Skilled in conflict resolution, with the ability to navigate challenging situations professionally and achieve constructive outcomes
  • Proven ability to prioritize tasks and meet deadlines, including flexibility to adjust work hours when necessary to meet program goals
  • Commitment to professionalism, consistently representing the agency with integrity, respect, and cultural sensitivity in all interactions

  • Knowledge of eligibility criteria and application processes for mainstream/entitlement programs helpful

Responsibilities

  • Meet with clients approved for RSP once a month and in crisis situations when necessary and make referrals as needed
  • Map out and enforce weekly a budget
  • Ensure all necessities are met and paid for (rent, food, utilities, medical care)
  • Communicate directly with landlords, bill providers, etc. on behalf of client
  • Provide financial advice and recommendations for proper spending habits
  • Work with new clients to complete paperwork, which identifies Bethesda House as the rep payee
  • Complete accurately and timely HMIS data entry
  • Provide face-to-face and telephone client engagement
  • Attend meetings as assigned and represent the agency in various community meetings
  • Maintain interactions/relationships with guests within the confines of appropriate professional boundaries and follow confidentiality policies
  • Oversee accurate, real-time tracking and allocation of client subsidies, ensuring responsible stewardship of funding and compliance with program requirements
  • Partner with Finance on approvals, documentation, and monthly reconciliation to maintain alignment, accuracy, and timely processing of funds
  • Conduct client intakes and assessments, guiding application completion and submission to expedite access to housing resources
  • Monitor participant eligibility and progress throughout the funding period, making adjustments to ensure appropriate and effective use of subsidies
  • Optimize funding across housing and eviction prevention initiatives to maximize program impact and reach
  • Conduct housing inspections to ensure safe, stable, and compliant living environments for program participants
