This role involves completing initial and ongoing assessments, coordinating family meetings, developing and implementing case plans, and determining necessary services for children and families. The Case Manager will also assist families in establishing support networks, support kinship and foster families, and prepare them for permanency goals such as reunification or adoption. Crisis intervention, court system interaction, and detailed documentation are key aspects of this position. The role also includes supervisory responsibilities for family support workers and participation in group or individual supervision. An on-call rotation is required for urgent issues outside of regular working hours, involving availability, rapid response, problem resolution, potential travel, and documentation.
