About The Position

The Case Manager Registered Nurse is a work-from-home position requiring travel to visit members within Maricopa County, Arizona. The role coordinates, documents, and communicates all aspects of the utilization and benefit management program. It applies clinical critical thinking, evidence-based care, and medical necessity criteria to support clinically appropriate treatment. The Case Manager provides members with care coordination, support, and education using care management tools and resources.

Requirements

• At least 2 years of experience in a clinical setting.
• Active RN license valid in Arizona.
• Must reside in Maricopa County, Arizona.
• Ability to travel up to 25% of the time within Maricopa County, Arizona.
• Associate’s degree.

Nice To Haves

• At least 2 years of experience in case management.
• Strong organizational and time management skills.
• Ability to collaborate with internal and external partners.
• Demonstrated proficiency in Microsoft Office Suite, including Outlook and Word.
• Previous experience collaborating with medical professionals.
• Bilingual in English and Spanish.
• Bachelor’s degree in Nursing.
