The Recovery and Reentry Case Manager manages an assigned caseload of justice-involved adults with substance use disorders and co-occurring behavioral health conditions who are incarcerated within the Blue Ridge Regional Jail Authority and transitioning back into the community. The incumbent is responsible for conducting comprehensive needs assessments, developing individualized reentry and service plans, coordinating evidence-based treatment and recovery supports, and ensuring continuity of care from incarceration through community reintegration. The Recovery and Reentry Case Manager collaborates closely with correction staff, behavioral health providers, peer recovery specialist, probation and parole, healthcare providers, housing agencies, employers, and other community partners to reduce overdose risk, promote treatment, and coordinating medication-assisted treatment (MAT/MUD) continuity, addressing barriers related to housing, employment, transportation, benefits, and healthcare, monitoring participant progress, and maintaining timely, accurate, documentation in accordance with agency, funding, and regulatory requirements. This is a grant-funded position that may terminate with the ending of the grant funding.
