Requirements

  • Bachelor's degree in social, behavior or human services field from an accredited university; or Associate degree in social, behavior, human service or health-related field; or High school diploma/GED with 2 years of paid or unpaid experience with individuals with Intellectual Developmental Disabilities
  • Must be able to qualify as a Qualified Intellectual Disabilities Professional (QIDP)
  • Must have valid TX driver's license and be insurable

Nice To Haves

  • Preferred to have at least one (1) year of experience in working with individuals with Intellectual and/or Developmental Disabilities

Responsibilities

  • Performs Service Coordination to individuals such as preventing, and managing crisis situations
  • Performs assessments, monitors, assessing needs, service planning and coordinating and making appropriate referrals
  • Monitor and evaluate the effectiveness of care plans and make adjustments as necessary.
  • Maintain accurate and up-to-date client records and documentation.
  • Maintains regular contact with individuals served to monitor services and identify needs
  • Will identify gaps in services so individual served experiences a continuum of care to the greatest extent possible
  • Develops Initial and Annual Person Directed Plan and makes revision based on the needs of the individual
