About The Position

Mental Health Support Services (MHSS) is seeking a part-time Case Manager (Part C Service Coordinator) with experience in service coordination activities and with young children with developmental disabilities to join their dynamic and supportive team. This role provides early intervention supports and services to children aged 0-3 years old with developmental delays and their families, in family homes or other natural environments. The position utilizes knowledge of child development, assessment procedures, curriculum development, parent training, and individual program planning. The Case Manager will prepare complex records and reports, interview families using family-friendly techniques, provide resources and support, and maintain casework documentation. This position is part of an approved Career Development Plan (CDP) offering career progression and salary incentives based on performance, qualifications, and experience.

Requirements

  • Bachelor's degree in a human services field, such as psychology, sociology, rehabilitation counseling, or a related field
  • Two years of experience in a human services field
  • An equivalent combination of training and experience
  • Working knowledge of case management methods, practices, and procedures
  • Ability to interview clients to identify deficiencies in personal, social, economic and health needs
  • Ability to work in a multi-disciplinary team environment providing treatment and rehabilitation services
  • Ability to communicate effectively orally and in writing
  • Ability to operate a personal computer and related software and other standard office equipment
  • Ability to develop and maintain effective working relationships with internal and external customers
  • Must complete the Part C Certification Process as a Service Coordinator prior to assuming position
  • Current valid driver’s license and good driving record required
  • Based on the Virginia DMV point system, records must not reflect a total of six or more demerit points within the preceding twenty-four months, or a major violation of six demerit points within the preceding thirty-six months
  • Out of state driving records must be obtained by applicant and presented at time of interview
  • Records must reflect at least three years of history and be dated within thirty days of the interview date
  • Pre-employment drug testing, FBI criminal background check, and education/degree verification required

Nice To Haves

  • Employment experience in a Part C Local System and/or extensive knowledge of Part C regulations
  • Experience in service coordination activities with young children with developmental disabilities
  • Knowledge of Infant Mental Health

  • Utilizes thorough knowledge of child development, assessment procedures and curriculum development, parent training materials and approaches, individual program planning and teaching skills
  • Prepares complex records and reports
  • Interviews families using family friendly techniques and provides resources and support as needed
  • Prepares and maintains casework documentation as required in the delivery of services (e.g. case histories, assessments, treatment plans, progress notes, correspondence)
  • Performs other work as required
