About The Position

As a Case Manager, Psychogeriatric, you will be part of a team that co-ordinates a support network and linkages for older adult and seniors with serious mental illness and other challenges (e.g., addictions). A key component of this position is building a support network of community resources that will enable the individual to increase their independence, dignity, and self-respect. The role will require direct service and support to clients in area such as daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual’s needs. Bradford House provides specialized assisted living support to vulnerable older adults with complex needs. Bradford House responds to each resident’s needs, so they can live safely and in good health in the community as they age. Through our care, we aim to postpone for as long as possible, or eliminate, any need for institutional care. Our program offers 24/7 care through our specialized Personal Support Worker (PSW) team, providing such services as assistance with personal care, essential housekeeping, laundry services, medication support and escorts to appointments. LOFT’s Case Managers offer one-on-one support and provide care coordination, crisis intervention, system navigation and client advocacy. They connect clients with the services they need to become healthier, happier, and as independent as possible. Our team calls upon a variety of resources, including medical, financial, emotional and recreational supports. Mental, physical and spiritual health are improved by social interaction and the sense of belonging that comes from a welcoming and supportive community. Our life enrichment services offer residents opportunities to enjoy life, try new things, and become involved in their local community. The goal is to ensure that clients have access to interesting and stimulating activities and social opportunities through regular in-house events and group outings in the community. Bradford House offers healthy and delicious meals and snacks for residents and is able to accommodate certain special diet requests.

Requirements

  • Completion of degree in social services/human services or equivalent
  • Minimum 3 years of experience in providing support to vulnerable seniors and older adults with special needs including mental illness, addictions, and severe social isolation.
  • Group counselling related to mental health, addictions, and related issues.
  • Knowledge of community resources and support.
  • An understanding of harm reduction, recovery and psychosocial rehabilitation principles, the therapeutic relationship, and an ability to come to an agreement with client on goals/priorities.
  • Crisis intervention skills, with well-developed problem solving abilities and interpersonal skills
  • Good oral and written communication skills and computer literacy, including MS word and Excel.
  • Counselling, responding and assessment skills.
  • Creativity and flexibility to approaches.
  • Ability to work effectively as part of a team and interact with workers from a variety of agencies.
  • Understanding of cultural competency and framework

Nice To Haves

  • Completion of Gerontology is an asset.
  • Knowledge of other languages is an asset.

Responsibilities

  • Assists clients in the development and implementation of a care plan tailored to the client’s needs and issues designed to improve quality of life. Identify and refer to resources specific to those needs and issues.
  • Act as a liaison and client advocate among those involved with the client’s care including family, other caregivers, medical and other multi-disciplinary providers.
  • Conduct assessments such as RAI and OCAN and provide timely and accurate record keeping and reports as required.
  • Provide supportive counselling or referral and linkages to health care, programs, or service providers.
  • Provide crisis management/intervention and assist in the development of crisis plans for clients.
  • Develop and maintain respectful, trusting, and collaborative relationship with clients by applying Recovery-oriented and Psychosocial Rehabilitation principles in all interactions.
  • Maintain strict confidentiality and follow all other required policies and procedures.
  • Consistently interact with clients, staff, family members, other community agencies and partners in a professional manner that reflects favourably on the organization.
  • Attend and actively participate in all required training.
  • Liaise, collaborate and coordinate with other professionals, service providers and partners.
  • Complete other duties, from time to time, as assigned by the Coordinator and/or Program Manager.
